This per diem position offers flexible scheduling, immediate case placement, and local assignments , perfect for HHAs who want meaningful work close to home.

Position Overview

As a Certified Home Health Aide with Affirmed Home Care, you will provide one-on-one, client-centered care to help clients live safely, comfortably, and independently in their homes. This per diem role is ideal for HHAs seeking flexibility, consistent work, and professional support in their local community .

Key Responsibilities

• Assist clients with personal care and daily hygiene

• Provide medication reminders

• Support meal preparation and light housekeeping

• Offer companionship, engagement, and emotional support

• Promote client safety, dignity, and independence

Requirements

• Valid NYS HHA Certificate

• Minimum 1 year of HHA experience

• Authorization to work in the U.S.

• Driver’s license preferred

• Physical exam within the past year

• PPD or QuantiFERON within 1 year or chest X-ray within 5 years

• MMR immunization within the past 10 years
