Pay Range: $33 - $59/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Coordinate patient care activities to ensure appropriate utilization of clinical resources.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to develop and implement patient-centered care plans.
• Facilitate communication between interdisciplinary teams to improve patient outcomes and satisfaction.
• Support discharge planning by identifying post-discharge needs and coordinating with community resources.
• Review clinical documentation and ensure compliance with hospital and regulatory standards.
• Provide education and counseling to patients and families regarding treatment plans and available support services.
• Monitor quality indicators and participate in continuous process improvement initiatives.

Key Requirements and Technology Experience:

• Key Skills; BSN, BLS, experience as a Registered Nurse.
• Current Registered Nurse (RN) license in the State of Georgia.
• BSN strongly preferred.
• 2+ years of recent clinical nursing experience in an acute care or case management setting preferred.
• Strong communication, critical thinking, and organizational skills.
• Knowledge of care coordination, discharge planning, and community resources.
